? ? ? ? ? ? ?น้ำนมไม่พอกับการเลี้ยงลูกด้วยนมแม่ เป็นปัญหาของการหยุดเลี้ยงลูกด้วยนมแม่ โดยจากการศึกษาพบเป็นปัญหาเพียงร้อยละ 5 แต่มารดารู้สึกว่าน้ำนมไม่พอถึงร้อยละ 50¹ ในการศึกษาของประเทศไทย พบเป็นสาเหตุในการหยุดเลี้ยงลูกด้วยนมแม่ที่พบบ่อยรองจากการกลับไปทำงานของมารดา²

? ? ? ? ? ?ทารกน้ำหนักตัวน้อยกับการเลี้ยงลูกด้วยนมแม่ การที่ทารกมีน้ำหนักตัวน้อยจะมีความยากลำบากในการเลี้ยงลูกด้วยนมแม่¹ โดยผลลบต่อการเลี้ยงลูกด้วยนมแม่จะมีหากทารกมีน้ำหนักตัวแรกคลอดน้อยกว่า 2500 กรัม² ปัจจัยที่จะช่วยในการเลี้ยงลูกด้วยนมแม่ในทารกที่น้ำหนักตัวน้อย คือ การเริ่มให้อาหารทางปากเร็ว³ การให้ทารกดูดจากเต้าหลังการปั๊มนมออก (non-nutritive sucking)⁴ และการให้ผิวทารกสัมผัสกับมารดาในลักษณะเหมือนจิงโจ้ (kangaroo care)⁵

? ? ? ? ? ? ? ทารกคลอดก่อนกำหนดกับการเลี้ยงลูกด้วยนมแม่ ในทารกที่คลอดก่อนกำหนดจะมีปัญหาในด้านการดูดและการกลืนซึ่งส่งผลต่อการเริ่มการเลี้ยงลูกด้วยนมแม่^1,2และมีความสัมพันธ์กับระยะเวลาในการเลี้ยงลูกด้วยนมแม่³ พบในทารกคลอดก่อนกำหนดเล็กน้อย (late preterm) มีความเสี่ยงในการเลี้ยงลูกด้วยนมแม่ยากเพิ่มขึ้นเป็น 1.72 เท่า (95%CI 1.24-2.38)⁴ โดยปัจจัยที่จะช่วยเพิ่มอัตราการเลี้ยงลูกด้วยนมแม่ในทารกคลอดก่อนกำหนดคือ การเริ่มปั๊มนมก่อน 12 ชั่วโมงหลังคลอด สำหรับการป้อนนมด้วยถ้วย⁵ การให้ทารกดูดจากเต้าหลังการปั๊มนมออก (non-nutritive sucking)⁶ การตรวจทดสอบน้ำหนักของทารก (test-weighing) และการงดการใช้จุกนมเทียมจะช่วยในการส่งเสริมการเลี้ยงลูกด้วยนมแม่อย่างเดียว⁷

? ? ? ? ? ? ?ภาวะลิ้นติดกับการเลี้ยงลูกด้วยนมแม่ ส่งผลให้เกิดการเจ็บเต้านม การเข้าเต้าไม่ดี ทารกน้ำหนักขึ้นไม่ดีและการหยุดนมแม่เร็ว ในประเทศไทยพบภาวะลิ้นติดร้อยละ 13.4 ของการคลอด¹ มีการศึกษาพบว่าทารกที่มีภาวะลิ้นติดจะได้รับการส่งต่อเพื่อรับการรักษาด้วยสาเหตุของการเจ็บเต้านมของมารดาร้อยละ 36-89^2,3 โดยเมื่อได้รับการรักษาด้วยการผ่าตัดแล้ววัดคะแนนการเจ็บเต้านมของมารดาที่ทารกมีภาวะลิ้นติดดีขึ้นราวร้อยละ 44-95^2,4-6 ทารกที่มีภาวะลิ้นติดมีการเข้าเต้ายากร้อยละ 25 เทียบกับในทารกปกติพบร้อยละ 3⁷ และมีคะแนนการเข้าเต้าต่ำกว่าทารกปกติอย่างมีนัยสำคัญ¹ โดยเมื่อเปรียบเทียบคะแนนการเลี้ยงลูกด้วยนมแม่ (infant breastfeeding assessment tool) และคะแนนการเข้าเต้าในทารกที่มีภาวะลิ้นติดก่อนและหลังได้รับการผ่าตัดรักษาพบว่ามีความแตกต่างอย่างมีนัยสำคัญ^5,6,8 ทารกที่มีภาวะลิ้นติดมีปัญหาเรื่องน้ำหนักขึ้นไม่ดีร้อยละ 16² การผ่าตัดแก้ไขเป็นทางเลือกในรายที่มีปัญหานี้ มีรายงานทารกที่มีภาวะลิ้นติดหลังได้รับการผ่าตัดรักษามีน้ำหนักดีขึ้นร้อยละ 65⁵ สำหรับการหยุดนมแม่เร็ว พบความเสี่ยงในการหยุดเลี้ยงลูกด้วยนมแม่ของทารกที่มีภาวะลิ้นติดในสัปดาห์แรกเพิ่มขึ้นถึงสามเท่า⁹

? ? ? ? ? ? ? ? ? ?ทารกมีความผิดปกติของช่องปากและความแข็งแรงของกล้ามเนื้อกับการเลี้ยงลูกด้วยนมแม่ ทารกที่มีความผิดปกติของช่องปากที่มีผลต่อกลไกการอ้าปาก การเข้าเต้า และการดูดนมมีผลต่อการเลี้ยงลูกด้วยนมแม่ ทารกที่มีปากแหว่งเพดานโหว่มีผลต่อการเข้าเต้าและการดูดนมของทารก โดยทารกที่มีเพดานโหว่จะมีปัญหาเรื่องการดูดนมโดยจะสร้างแรงดูดที่เพียงพอในการดูดนมไม่ได้¹ นอกจากนี้ทารกที่มีคางเล็กหรือลิ้นใหญ่คับปากอาจมีความลำบากในการเข้าเต้าด้วย เช่น ทารกกลุ่มอาการดาวน์ ในทารกที่มีความผิดปกติของความแข็งแรงของกล้ามเนื้อที่ส่งผลทำให้กล้ามเนื้ออ่อนแรงอาจมีต่อการอ้าปาก อมหัวนมและการดูดนมเช่นเดียวกัน
