Case: ผู้หญิงตั้งครรภ์ไทยอายุ 22 ปี ? G₂P₀A₁? GA 37⁺³ wk by LMP

CC: มาตามนัด OPD ANC มีอาการปวดท้อง

 

S: ผู้ป่วยรู้สึกตัวดี อาการทั่วไปปกติดี มีอาการเจ็บแผลผ่าตัดอยู่เล็กน้อยเวลาขยับตัว มีอาการหน้ามืดเวลาลุก น้ำคาวปลาเป็นสีแดง ใช้ผ้าอนามัย 3 แผ่น ชุ่ม น้ำนมเริ่มไหลดี ทารกกินนมได้ดี ยังใส่สายสวนปัสสาวะ ยังไม่ถ่ายอุจจาระ ไม่มีไข้ ไม่มีภาวะซึมเศร้า

 

O:? V/S BT: 37.2 C????? BP: 102/72 mmHg????? RR: 20/min????? PR: 100 bpm

GA: A Thai female, alert, not pale, no jaundice, looks well

HEENT: not pale conjunctivae, anicteric sclerae

Heart: regular rhythm, full pulse, normal S₁S₂, no murmur

Lungs: equal chest expansion, good air entry, equal breath sound both sides, clear breath sound, no adventitious sounds

Breast: No engorgement, no signs of inflammation, no ulcer, hyperpigmentation of nipple and areola, symmetrical

Abdomen: distended, normal active bowel sound, no abdominal tenderness, liver and spleen cannot be palpated, fundal Height at umbilicus, normal surgical scar

Extremities: no pitting edema, capillary refill <2 seconds

Neurologic Examination: grossly intact all, Deep tendon reflex 2+ all

Problem list

1. Postpartum C/S day 1

A: เมื่อวานได้ set OR for emergency C/S due to secondary arrest of dilatation เวลา 17.00 น. No intraoperative complication, estimate blood loss 500 ml คลอดทารก term male, BW 2630 gm., Apgar 10-10-10. Intraoperative finding: พบว่าไม่มี left fallopian tube ตรงกับประวัติที่ผู้ป่วยให้ว่าเคยตั้งครรภ์นอกมดลูกและตัดท่อนำไข่ข้างซ้าย. no immediate postoperative complication วันนี้อาการทั่วไปปกติดี มีอาการหน้ามืดเวลาลุกนั่ง น้ำนมไหลดี ตรวจร่างกายปกติดี

P: – Routine postpartum care

-????????? Plan off Foley?s catheter + observe voiding (if not void in 6 hr, please notify)

-????????? Off IV fluid

-????????? Soft diet

-????????? Encourage breast feeding

-????????? Paracetamol (500) 2 tab po prn ทุก 4-6 ชม.

-????????? Advice ให้ค่อยๆลุกนั่งช้าๆ อาการหน้ามืดจะค่อยๆดีขึ้น

S: ผู้ป่วยรู้สึกตัวดี อาการทั่วไปปกติดี มีอาการเจ็บแผลผ่าตัดอยู่เล็กน้อยเวลาขยับตัว ไม่ได้ขอยาแแก้ปวด ไม่มีอาการหน้ามืดแล้ว น้ำคาวปลาเป็นสีแดง ลดน้อยลง กินข้าวได้ น้ำนมไหลดี ทารกกินนมได้ดี ปัสสาวะออกปกติ สีเหลืองใส ยังไม่ถ่ายอุจจาระ ไม่มีไข้ ไม่มีภาวะซึมเศร้า

 

O:? V/S BT: 37.5 C? ????BP: 110/70 mmHg????? RR: 16/min????? PR: 72 bpm
