ยา Tenofovir สามารถผ่านรกได้โดยมีระดับยาในกระแสเลือดมารดาเท่ากับระดับยาในเส้นเลือดในสายสะดือทารก1 ยานี้จัดอยู่ใน category B ของยาที่ใช้ระหว่างตั้งครรภ์ มีการศึกษาถึงความพิการในทารกที่มารดาได้รับยา Tenofovir ระหว่างตั้งครรภ์ในไตรมาสแรกพบความพิการในทารกร้อยละ 2.3 สำหรับการให้ในระหว่างการตั้งครรภ์ในไตรมาสที่สองและสามพบความพิการในทารกร้อยละ 2.3 ซึ่งอัตราการเกิดความพิการในทารกที่พบนี้ไม่แตกต่างจากมารดาที่ไม่ได้รับยาต้านไวรัส ในกรณีที่ให้ขนาดสูงพบว่ามีผลต่อแร่ธาตุในกระดูกลดลง การเจริญเติบโตผิดปกติ และลดระดับของ insulin-like growth factor1 ซึ่งจะกลับมาเป็นปกติเมื่อหยุดยา2-5 บางรายงานพบว่ามีน้ำหนักของทารกที่ได้รัยยานี้ที่อายุหนึ่งปีน้อยกว่า6 ดังนั้น ยาTenofovir จึงเป็นทางเลือกในกรณีที่จะใช้ยาต้านไวรัสระหว่างการตั้งครรภ์
